EOS (<72 h): vertical transmission, GBS and gram-negatives, empirical benzylpenicillin + gentamicin.
LOS (>72 h): nosocomial, CoNS and Staphylococcus aureus dominate, empirical vancomycin + gentamicin.
In both: obtain blood culture before antibiotics, but never delay treatment pending the result.
LP should be performed in all confirmed or probable sepsis cases unless the neonate is too unstable.
