Sarnat grading of HIE (modified). Therapeutic hypothermia (TH) is indicated for
Grade II and III in neonates ≥36 weeks, provided criteria are met and cooling begins within 6 hours of birth.
Grade I (mild HIE) generally has a good prognosis and TH is not currently standard of care.
Grade III carries the highest mortality and risk of severe neurodisability even with TH.
